CMS Five-Star Quality Rating System

A Five-Star rating represents national recognition for delivering high-quality, person-centered care, and we are proud to consistently meet—and strive to exceed—these benchmarks.

The Centers for Medicare & Medicaid Services (CMS) created the Five-Star system to help families evaluate nursing homes and understand key indicators of resident well-being. Ratings are publicly displayed on the federal Care Compare website and are updated regularly using the most current validated data.

How CMS Calculates the Five-Star Rating

CMS assigns an overall rating based on performance across three core areas: health inspections, staffing, and quality outcomes.

Health Inspection Rating

This rating is based on unannounced, comprehensive inspections that review resident safety, quality of care, and compliance with federal standards. CMS considers the two most recent standard surveys, along with three years of complaint and infection control inspections. The most recent findings carry the greatest weight, giving families a clear view of current performance.

Staffing Rating

Staffing ratings measure how well a facility is staffed to meet the clinical and daily needs of residents. CMS evaluates payroll-verified staffing hours for registered nurses, licensed nurses, and nurse aides, as well as weekend staffing patterns and overall staff stability. Because high-quality care relies on consistent, well-trained teams, staffing contributes significantly to the overall rating.

Quality Measures (QM) Rating

Quality Measures reflect resident outcomes and overall quality of life. CMS analyzes both clinical assessments and claims data to understand how residents are doing over time. This includes factors such as fall rates, pressure ulcer prevention, functional improvement, medication use, hospital readmissions, emergency department visits, and successful return-to-home outcomes. CMS evaluates both long-stay and short-stay residents to form a complete picture of care quality.
